Why the Map Is Changing
The rapid expansion of dry needling legislation is not happening in a vacuum. Physical therapy associations — particularly the American Physical Therapy Association (APTA) — have actively lobbied state legislatures to authorize dry needling under existing PT licenses. These efforts have been successful in most states.
What is often missing from these legislative debates is adequate input from the acupuncture community. State acupuncture boards frequently have limited political resources compared to the well-funded PT lobby. The result: laws that allow minimal-training needling practitioners to operate without accountability to the acupuncture standards developed over centuries.
The concern is not that physical therapists cannot learn to needle safely. The concern is that 27–54 hours of training — conducted under a PT license that was never designed for needling — creates a system where patients cannot accurately assess what they are receiving or from whom.
What States With Restrictions Show Us
States where dry needling remains restricted or contested typically have stronger acupuncture advocacy organizations, more engaged state acupuncture boards, and legislators who have heard from their licensed acupuncturist constituents. This demonstrates that advocacy works — and that the acupuncture community must remain organized and vocal.
